'No Botox, No Fillers?' - The Celebrity Skincare Myth We Need to Talk About

Every few months, a new celebrity comes out and insists they’re 'all natural' - no Botox, no fillers, just water, sleep, and a 'really good skincare routine.' The latest to join the conversation? The beautiful Hailey Bieber, who recently declared on the In Your Dreams With Owen Thiele podcast that she doesn’t have Botox - except in her jaw, for TMJ (temporomandibular joint disorder).
Let’s pause there.
Because here’s the thing - Botox for TMJ is Botox. It’s medical, yes. But it also just so happens to slim the lower face, soften a square jawline, and create that delicate, feminine contour many of us recognise as the 'Hailey look.' So while she might not be freezing frown lines yet, the idea that she’s completely untouched is... well, a little misleading.
The Botox Paradox
Hailey is only 28 - she's young, genetically blessed, and has access to the world’s top dermatologists, nutritionists, facialists, and lasers. At that age, and with access to the best preventative aging treatments - of course she doesn’t need three areas of Botox yet. What she is doing, however, is actively treating and slowing down the ageing process before it’s even started.
In her own words, Hailey is 'really diligent with her skincare,' using a cocktail of high-end and clinical products - and likely spending more on facials each month than most people do in a year. She also openly admits to regular PRP and PRF injections (treatments that use her own blood to stimulate collagen and elastin production), along with 'light little lasers' a few times a year. The issue is that Hailey’s idea of skincare isn’t what most people mean when they use the term. When an everyday woman hears 'skincare,' she imagines cleansers, serums, and SPF - not needles, lasers, and £800 facials. Hailey’s version of 'skin care' is, in reality, a schedule of in-clinic aesthetic treatments with a hefty price tag - results that no over-the-counter serum can replicate.
In terms of other injectables, she's been questioned online about possible lip filler, which she has denied, though to our expert eye, it looks more like a subtle lip flip - a small amount of Botox placed around the upper lip to gently lift and evert it. It gives that soft, naturally full look without adding volume, which fits perfectly with her minimalist aesthetic.
Just to be clear - we’re not shaming her at all. We're all for preventative treatments, natural looking tweakments and ageing on your own terms. Hailey is stunning. She looks fantastic. There’s no judgement here - it’s simply that what’s being presented isn’t the whole truth.
We live in an online world where perception is everything, and as women, we’re constantly told that unless we can afford a full suite of advanced, preventative treatments in our 20s, we’re somehow falling behind. We're tricked into thinking we can achieve the same results with a moisturiser alone. Feel like failures when no 'magic ingredient' works. The 'ew, I don't use Botox' attitude only adds to that pressure - it’s like saying, 'I just use supplements and meditation for my mild depression' to someone who genuinely needs medication. Different needs, different stages, different tools and in the case of aesthetic treatments, different budgets - and there's no shame in any of them.

The Truth About 'Natural' Celeb Skin
When celebrities say they don’t 'do Botox,' it often leaves the public feeling confused or inadequate - like glowing, line-free skin is achievable with just an ice roller, gua sha and a £15 serum. But the truth is, most of what they do isn’t accessible or practical for the average person. Multiple laser sessions, PRP, PRF, and facials every few weeks add up - in both down time and cost.
And if you’re 35 and starting to notice deeper frown lines or crow’s feet, 'light little lasers' simply aren’t going to erase them. Those treatments work beautifully for maintenance and prevention, but once wrinkles have settled in, you need a little extra help - and that’s where neuromodulators like Botox come in.
If you’re not short of time or money, then by all means - follow the celebrity playbook. Do the regular facials, the lasers, the PRP, the ice baths, and the lymphatic drainage massages. But if you live in the real world, where results and practicality matter, a little well-placed Botox and a regular skin booster like Profhilo can achieve in minutes what months of expensive maintenance can’t.
Honesty Matters
At our East London aesthetics clinic, we’re not anti-celebrity or anti-Hollywood glow - quite the opposite. We love when people invest in their skin. But we are pro-transparency. Botox, fillers, skin boosters, microneedling, PRF - they’re not dirty words. They’re all tools that, when used subtly and professionally, help you look fresh, natural, and confident. One thing we can agree on though, is that the best results don’t look 'done' - they just look like you, on a really good day.
So, while Hailey’s 'no Botox yet' stance makes for a great headline, let’s not forget that she’s also benefiting from some of the most effective (and expensive) preventative treatments out there. And TMJ botox, whether for medical reasons or not, does still have a pleasing aesthetic side effect.
The real takeaway? You don’t need a celebrity budget to age gracefully - just the right advice, the right treatments, and a little honesty about what really works.
